1. The “Put Your Trust in OPTO+” contest is held by OSI Group in partnership with the OPTOPLUS clinics across Quebec. Contest is open to all Quebec residents 18 years of age and over. Contest is not open to OSI Group employees, agents and representatives, affiliates, subsidiaries, parent or related companies, advertising and promotional agencies, suppliers of prizes, material and services related to this contest, members of their immediate families (brothers, sisters, children, father, mother), their legal or de facto spouses or persons with whom they are domiciled. In order to be eligible, online form must be correctly completed and include name, email address and answer to the skills testing question (100-25) x 2. Copies and/or facsimiles are not accepted.
2. Contest entries can be submitted online during the referral process throughout the 2 contest periods at www.optoplus.com/en/promotions/referral-program/. No purchase necessary. Online forms must be completed prior to:
– 2024-06-13 at 11:59 p.m. (EST) for contest #1
– 2024-11-21 at 11:59 p.m. (EST) for contest #2
under pain of absolute nullity. Each reference given is eligible for a contest entry; multiple entries are therefore accepted under the condition that a different person is referenced in each entry.
3. Contest closes:
– 2024-06-13 at 11:59 p.m. (EST) for contest #1
– 2024-11-21 at 11:59 p.m. (EST) for contest #2
4. Draw will take place on:
– 2024-06-14 at 11:00 a.m. (EST) for contest #1
– 2024-11-22 at 11:00 a.m. (EST) for contest #2
From among all eligible entry forms received. To be declared a winner, the person whose entry is selected must correctly answer the online skills-testing question (100-25) x 2. Winner will be notified by email immediately following the draw. Failure to meet any of the above-mentioned conditions will result in disqualification. In such a case, a new draw will be held until a new winner is declared.
4.1One gift certificate is to be drawn during each contest period (2 contest periods, therefore, a total of 2 prizes). Prize must be accepted as awarded and cannot be redeemed for cash or exchanged. 2 gifts certificates, each valued at $1000, will be drawn for a total maximum value of $2000. This gift certificate is only valid in all OPTOPLUS clinics throughout Quebec.
5. Random draw will take place on:
– 2024-06-14 at 3:00 p.m. (EST) for contest #1
– 2024-11-22 at 3:00 p.m. (EST) for contest #2
at OSI Group’s head office located at 4 Place du Commerce, Suite 200, Montreal QC, H3E 1J4, amongst all ballots received for each of the 2 contest periods.
6.Contest organizers will contact the winner by email immediately following the random draw.
7.The location, date and time by which the prizes must be claimed or, as the case may be, the fact that the prizes are sent to the winners electronically by email; each gift certificate will be sent to each of the winners by email from the OSI Group head office, located at 4 Place du Commerce, Suite 200, Montreal QC, H3E 1J4 by a person mandated by the company to the email address provided by the winners.
8. A jury will not select the winners.
9.Any person for whom a publicity contest is carried on, his employee, representative or agent, a member of the jury and the person with whom they are domiciled may not enter the contest. Contest is open to all Quebec residents 18 years of age and over. 10..“Any litigation respecting the conduct or organization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.”
11.A winner is in no way required to submit to skill-testing to claim their prize, no purchase required.
